LillyDirect, TrumpRx, and NovoCare: Can You Really Get Cheap GLP-1s With a Prescription From Your Doctor?

LillyDirect: Zepbound Direct From Eli Lilly

LillyDirect is Eli Lilly's direct-to-patient platform for Zepbound (tirzepatide). As of early 2026, self-pay pricing through their platform starts at $299 per month for the 2.5 mg starter dose, $399 for the 5 mg dose, and $449 for maintenance doses of 7.5 mg through 15 mg when you refill within 45 days through their Self Pay Journey Program.

Those are real prices, and they represent a massive drop from Zepbound's list price of over $1,000 a month. But there are a few things worth understanding before you assume this is your best option.

LillyDirect is a closed ecosystem. The platform connects you with affiliated telehealth providers and its own pharmacy fulfillment service. You generally cannot take a prescription from your own primary care doctor and get LillyDirect pricing at your local pharmacy. The discounted pricing is tied to using their pipeline from start to finish.

Refill timing matters. The best pricing on higher doses requires you to refill within 45 days of your previous shipment. If you miss that window, you lose the promotional rate and pay the standard price, which is significantly higher.

Insurance is not part of the equation. LillyDirect self-pay purchases do not count toward your insurance deductible or out-of-pocket maximum. You are paying entirely out of pocket, which means there is no accumulation of benefit toward your plan limits.

NovoCare Pharmacy: Wegovy and Ozempic Direct From Novo Nordisk

Novo Nordisk launched NovoCare Pharmacy in March 2025 as a direct-to-patient delivery option for Wegovy (semaglutide). The standard self-pay price is $349 per month for most doses, down from the list price of $1,349.

Novo also rolled out limited-time introductory pricing in November 2025. New self-pay patients can access the two lowest doses (0.25 mg and 0.5 mg) of Wegovy or Ozempic for $199 per month for their first two months through March 31, 2026. After that, pricing reverts to $349 per month. The new oral Wegovy pill is available at $149 per month for the 1.5 mg and 4 mg doses through April 15, 2026.

The same structural limitations apply here as with LillyDirect.

It is designed as a direct channel. Your prescriber sends the prescription to NovoCare Pharmacy specifically. The savings pricing is not something you can take to CVS or Walgreens with an outside script and expect to receive.

Government beneficiaries are excluded. Patients on Medicare, Medicaid, TRICARE, or VA benefits are not eligible for self-pay savings offers from Novo Nordisk, even if they choose to pay cash.

Introductory pricing is temporary. The $199 starter offers expire, and the $149 oral pricing has its own expiration date. After the promotional window closes, you are looking at $349 per month for injectable Wegovy.

TrumpRx: The New Government Platform

TrumpRx.gov launched on February 5, 2026, as part of the Trump administration's Most Favored Nation pricing initiative. The platform is a portal that directs patients to manufacturer websites where they can purchase medications at negotiated prices. It does not sell drugs directly.

For GLP-1 medications specifically, the advertised prices are significant. The monthly cost of injectable Wegovy and Ozempic drops from over $1,000 to an average of $350 and as low as $199 depending on dose. Zepbound pricing averages $346, starting at $299. The oral Wegovy pill is listed at as low as $149 per month.

Here is where it gets complicated.

TrumpRx is a cash-pay platform. Purchases made through TrumpRx do not count toward insurance deductibles or out-of-pocket maximums. If you have commercial insurance that covers GLP-1s, you may actually pay less going through your plan.

The prices are measured against list price. The headline discounts compare TrumpRx prices to the full retail list price, which most insured patients already do not pay. For uninsured or underinsured patients, these are meaningful savings. For patients with decent pharmacy coverage, the math is less clear.

Medicare and Medicaid coverage is still being worked out. The administration announced that Medicare will begin covering GLP-1s for obesity starting mid-2026 at $245 per month with a $50 patient copay, but this is being implemented through demonstration programs that have not fully launched yet.

No clinical support is built in. TrumpRx connects you to the medication, not to a care team. You still need a prescriber, labs, dosing guidance, and ongoing monitoring from somewhere else.

Can You Just Get a Prescription From Your Doctor and Fill It at the Pharmacy?

Your primary care doctor can absolutely write a prescription for Wegovy, Ozempic, or Zepbound. But here is what typically happens next.
